Family Connection TN Voices for Children

 
 
 

An intensive, in home service focusing on building the skills and supports of the child/youth/family to prevent placement outside the home, stabilize current placement, or facilitate reunification/permanency in the community. The program builds resiliency by equipping the child/family with the knowledge/skills to avoid future crises, developing parenting skills in managing their child/youth in the home, and assisting the child/youth in learning to manage their illness for long term success.

Eligibility Requirements

The Family Connection serves children/youth and their families that display the following risk factors: primary diagnosis of mental illness or evidence of a severe emotional disturbance (SED), and high risk of placement in an out of home therapeutic setting or the possibility of returning from an out of home therapeutic setting with supports in place. The program is currently available to DCS involved children/youth in Davidson County, through TennCare contracts or single case agreements with managed care organizations, and potentially through the Direct Purchase Authority in the DCS regions.
