Primary care services are provided to uninsured adults ages 19 to 64 in 56 county health department locations. Services include wellness exams, screenings, management of chronic medical conditions and referral for specialty care when needed. Family planning, immunizations and Women, Infants and Children (WIC) services are included. Charges are based on sliding scale.
